Please inform your doctor if the patient:

• Has Severe Liver Disease: CRITICAL. Ranolazine is processed by the liver.¹² Patients with severe liver impairment should not take this drug.

   

• Has Kidney Disease: Especially moderate to severe kidney impairment, as dosage may need adjustment.¹³

   

• Has Specific Heart Rhythm Issues: Ranolazine can prolong the QT interval on an ECG. Inform your doctor if you have a history of Long QT Syndrome or irregular heartbeats.

• Is taking other medications: CRITICAL. Ranolazine interacts with a large number of drugs, including certain antifungals (Ketoconazole), antibiotics (Clarithromycin), HIV medications, and other CCBs (Verapamil/Diltiazem).¹⁴ These combinations can dangerously increase the level of Ranolazine in the blood.

Guidelines for Effectiveness

• Dosage: Take exactly as prescribed, usually one tablet (500mg) twice a day (morning and evening). Your doctor may increase the dose to 1000mg twice daily based on your response.

• Timing: Take your doses approximately 12 hours apart to ensure 24-hour symptom control.

• Administration: Swallow the tablet whole with a glass of water.

• Do Not Crush/Chew: CRITICAL. Do not crush, break, or chew the extended-release tablet. Doing so will release the entire dose at once, which can increase side effects and disrupt the all-day effect.

• Food: It can be taken with or without food.¹⁶

   

• Avoid Grapefruit: CRITICAL. Do not consume grapefruit or grapefruit juice, as it can significantly increase the level of Ranolazine in your blood, raising the risk of toxicity.¹⁷

   

• If You Miss a Dose: Take it as soon as you remember. If it is almost time for your next dose (within 6-8 hours), skip the missed one. Never double the dose.

• Storage: Store at room temperature away from moisture and direct heat.¹⁸
